How to Fill Application Form

Assuming that you have already registered, just login and follow the instructions as below to fill in the form.

Follow these steps to Fill the Application Form :

Step 1 : Login with your Email-id & password. Then click on the Application. Next click on Application Form.

Step 2 : Select the Exam center that you prefer & select the Intended Month. Intended Month will be the month in which you will be giving your exam, not the month of booking. Select the Fresher option & then click Next at the bottom right corner.




Step 3 : Fill in all the Personal Details, upload a photo of you. After filling all the details correctly, click Next.




Step 4 : Fill in all the details correctly. If you dont have have an EXN-45(for people giving exams in Mumbai MMD), no need to worry, just leave the option blank, and fill the remaining details correctly and click Next.




Step 5 : Fill your Contact Details correctly and click Next.




Step 6 : Fill in the fee details correctly. People giving exam from Mumbai MMD, will have to leave this section blank. Click Next to continue.




Step 7 : Select the proper stream & Education Level. Click Next to continue.




Step 8 : Fill the proper details of Pre-Sea Training Course. Institute INDOS no. can be obtained fron DG Shipping site, or click here. Click Next to continue.




Step 9 : In this section, you have to fill PHASE 2 row.

Date of Completion will be your Sign-Off Date.
Designated Training Officer will be the Chief Engineer when you signed off.
Details of Certificate of Competency will be COC no of that Chief Engineer.
Fill in the proper details and clcik Next to continue.





Step 10 : In this section, you will have to fill your sea experience.

No. of Testimonials will be the total no. of testimonials for a single ship. Ex. if you have sailed on  2 ships, and have 3 testimonials, 2 from first ship and 1 from another, then, in the first row, no. of testimonials will be 2. Then add a row, and the no. of testimonial will be 1.
Fill the details of different ships accordingly.
In the column Date, From-To will be the sign on and sign off dates for that particular ship.
Article period will come automatically. 
QSS will be the propulsion time, i.e. the actual no of days spent in sea. If you have more than 1 testimonial for the ship, then add the total propulsion days of the testimonial and write the total.
Total QSS will be the total no. of propulsion time, of all the ships.
Fill the details correctly and click Next to continue.




Step 11 : Fill the details of all the STCW courses, except ERS course details.
STCW Code A will be given on the certificate.
Fill in the details of FPFF, PST, PSSR, EFA, MFA, AFFC & PSCRB course details.

Fill the details and click Next to continue.




Step 12 : Select the date when you are going to submit the application. this can be filled later. Select Functions 3, 4B, 5 & 6.
Select I AGREE, click Next to continue.




Step 13 : Fill this only if you had a COC earlier. If no, just leave everything blank and click Next to continue.

Step 14 : Fill the ERS details in this section. In the Remarks section write PASS if completed the course successfully. Click Next to continue.




Step 15 : This is the final step for submitting the form. Once you click on the Submit option, you will not be able to edit any details that you have provided or that are remaining to be filled.
You can submit the application form any time later, all the details will be saved, so you can logout at anytime and login later with the same details.
Once you submit the form, it will go for review in the MMD and you will get a confirmation mail once your application is approved. Then you can book the corresponding seat for exam.






After submitting the application, you can take printouts. But the pdf file will not open from Chrome browser, you will have to open it in Internet Explorer.

How To Register

The first thing you will have to do for applying for Class  IV  exam, is to register online on E-Pariksha site. Most of the times the link is down and cannot be accessed. Here is a link for E-Pariksha :

Follow the steps to Register & fill the application form :

Step 1 : After opening the page, you will have to register :

Step 2 : Next fill in the details and click on Register :



Step 3 : Login :

After successfully registering, you will get a confirmation mail on the e-mail id provided for registration. 
In the mail, you will receive the password and login-id.
Now open the link again and enter the login-id and password and click on Login.


Class IV Requirements

        Before directly applying for Class  IV, one should be certain that he/she has all the documents ready beforehand, in order to avoid unnecessary cancellation or delay of seat booking. Following are the documents compulsorily required for applying:


  1. CDC with ship stamp.
  2. Passport.
  3. Indos.
  4. TAR Book, with appropriate signs and stamps wherever necessary.
  5. Sea-Service Certificate from the company.
  6. Valid Medical Certificate.
  7. Testimonials From Chief Engineer.
  8. 10th Passing Certificate.
  9. (10+2) Passing Certificate.
  10. Diploma Certificate (if applicable).
  11. Degree Certificate.
  12. All Semesters Marks Card.
  13. Pre-Sea Training Course Degree Certificate.
  14. EFA (Elementary First Aid) Certificate.
  15. PSSR (Personal Safety & Social Responsibilities) Certificate.
  16. PST (Personal Survival Techniques) Certificate.
  17. FPFF (Fire Prevention & Fire Fighting) Certificate.
  18. MFA (Medical First Aid) Certificate.
  19. AFFC (Advanced Fire Fighting) Certificate.
  20. PSCRB (Proficiency in Survival Crafts & Rescue Boats) Certificate.
  21. ERS (Engine Room Simulator) Certificate (Post-sea Certificate).
  22. Application Form Print.

        Out of the above Certificates, only Sea-Service & Valid Medical Certificates have to be submitted in original for verification. Rest all certificates' self attested photocopies have to be submitted. But during verification, both original & photocopies have to be carried as they will be checked & compared.

        During the form filling process, you will need to carry Passport size photographs with you, carry atleast 4.
